Hormone Replacement Therapy (HRT) is a topic that frequently comes up at The Wellness Loft, and understandably so – it’s also one surrounded by a lot of confusion and historical baggage. In this article, our goal is to provide clarity, specifically around Bioidentical Hormone Replacement Therapy (BHRT), explaining what it is, how it differs from older synthetic forms, safety considerations, and the modern, precise ways we can administer it.

What’s the difference between bioidentical and synthetic hormones?

This is perhaps the most crucial distinction to understand. Bioidentical Hormone Replacement Therapy (BHRT) utilizes hormones that are chemically and structurally identical to the ones your own body naturally produces – think estrogen, progesterone, and testosterone. Even though they might be derived from plant sources like wild yams, they are processed to be an exact molecular match. This identical structure is key because it allows our bodies to recognize and utilize these hormones effectively, just as they would our own. As we often explain to patients, the body doesn’t have to spend any extra effort trying to figure out how to convert or use them, since bioidentical hormones are the most readily assimilated form we can use.

Synthetic hormones, on the other hand, are chemically altered compounds developed in a lab. While they are designed to mimic the effects of our natural hormones, their structure is different. This structural difference means they might bind to receptors differently, break down into different metabolites, and potentially lead to different effects than the bioidentical options. If you are in need of hormone replacement, it’s important to work with someone who truly understands bioidenticals and their unique properties.

What are the benefits of taking bioidentical hormones?

When hormone levels decline or become imbalanced, it can lead to a wide array of symptoms that significantly impact our quality of life. BHRT, when appropriately prescribed, aims to restore optimal levels and alleviate these symptoms. Some potential benefits include:

  • Symptom Relief: This is often the primary goal – easing symptoms associated with perimenopause and menopause like hot flashes, night sweats, sleep disturbances, mood swings, and vaginal dryness, or addressing symptoms of andropause (low testosterone) in men, such as fatigue, low libido, and changes in mood or muscle mass.
  • Improved Quality of Life: Beyond specific symptoms, balanced hormones contribute to overall well-being, potentially improving energy levels, cognitive function (like reducing brain fog), sexual health, and even supporting bone density.

When it comes to our health, safety is paramount and something that needs to be addressed with each and every patient considering hormone therapy. Many patients’ concerns stem from older studies, like the Women’s Health Initiative (WHI), which primarily used synthetic, non-personalized hormones (specifically oral conjugated equine estrogens and medroxyprogesterone acetate) in a specific population. The results of those studies highlighted certain risks, but depict an “apples-to-oranges” comparison. As such, it’s important not to extrapolate those results directly onto modern, personalized BHRT protocols using different hormone structures and administration routes.

The safety profile of contemporary BHRT depends heavily on several key factors:

  • Using Bioidentical Structures: Utilizing hormones identical to the body’s own is believed to offer a more natural interaction.
  • Individualized Risk Assessment: A thorough evaluation of personal and family medical history, lifestyle, and risk factors is essential before starting any hormone therapy.
  • Appropriate Dosing: The principle is always to use the lowest effective dose needed to achieve therapeutic goals and alleviate symptoms. We tend to shy on the conservative side in relation to hormone prescribing. The ultimate goal is to help our patients have enough circulating hormones to support their body’s optimal function, but not so much that we’re going to turn around and create an excess.
  • Route of Administration: How hormones are delivered matters. For instance, transdermal (through the skin) estrogen is often preferred over oral routes as it avoids a first pass metabolism in the liver, potentially reducing certain risks like blood clots.
  • Progesterone Use: For women with a uterus receiving estrogen, concurrent use of adequate progesterone is crucial for endometrial protection.
  • Regular Monitoring: Ongoing follow-up with symptom checks and periodic lab testing is vital to ensure the dose remains appropriate and safe over time.

When managed carefully by a knowledgeable provider who prioritizes these factors, personalized BHRT is considered a viable and potentially favorable option for many individuals seeking symptom relief and improved well-being. However, it is not without potential risks, and a thorough discussion is always necessary.

Delivery Methods: Why Precision Matters (and Why We Avoid Hormone Pellets)

How hormones are delivered is just as important as the hormones themselves. At The Wellness Loft, we prioritize methods that allow for precision, flexibility, and easy dose adjustments based on patient response and follow-up testing. This is why we have elected not to use hormone pellet therapy.

While pellets (small implants inserted under the skin) might seem convenient, we find they present significant challenges. It’s much harder to regulate how the body breaks them down, leading to variable absorption rates between individuals. Getting the dose accurate from the outset is difficult, and crucially, “once it’s in, it’s in.” Until the pellet wears off months later, there is no way to adjust the dose down if side effects occur, or up if the dose is not sufficient. We have also seen instances where the body encapsulates the pellet, further slowing absorption unpredictably. While newer pellet technologies exist, we find other methods offer superior control and safety.

Our preferred and precise BHRT methods include:

  • Topical Creams/Gels/Drops: Applied to the skin, usually via metered-dose pumps that deliver an exact amount with each pump. This allows for consistent absorption and easy dose adjustments.
  • Troches: These lozenges are placed between the cheek and the gums to allow absorption through the buccal mucosa and into the bloodstream, avoiding first pass metabolism in the GI tract.
  • Transdermal patches: These small adhesive patches are applied either once or twice weekly for slow and steady release of hormone through the skin.
  • Injectables: Often used for testosterone replacement, intramuscular or subcutaneous injections allow for very precise dosing administered weekly, bi-weekly, or daily. We use clean and appropriate carrier oils like MCT or almond oil for best dispersion.
  • Vaginal Applications: Creams or suppositories containing estrogen or DHEA can provide targeted relief for localized symptoms like vaginal dryness.

The ability to fine-tune the dosage with these methods is paramount. We can make small, precise adjustments based on your feedback and lab results, ensuring you get exactly what your body needs – no more, no less. This contrasts sharply with imprecise methods, like estimating amounts from a tube, which can easily lead to incorrect dosing.

Carly Cavaliere is a Board-Certified Physician Assistant and Licensed Massage Therapist who draws on her advanced training in anatomy, physiology, and kinesiology to provide intentional, functional, and therapeutic massage. He enjoys watching sports, starting his day with a nice cup of coffee, and cherishes the support of his amazing family and friends. Erin Connor is a Licensed Master Esthetician and Skincare Specialist with nearly a decade of success in the skin care industry. As the nurse at The Wellness Loft, she learns directly from Dr. Kimbi, Dr. Lyndsey, and Morgan while pursuing her Doctor of Nursing Practice at the University of Alabama at Birmingham, specializing in Women’s Health. Dr. Lyndsey DeStefano Chapman is a National Board Certified Family Nurse Practitioner, born and raised in Charleston. Lyndsey is passionate about spending genuine, quality time with her patients and forming lasting connections. Lyndsey trained under Dr. Kimbi for both her undergraduate and doctoral nursing degrees, and shares the same passion and joy of caring for individuals on a deeper level and getting to the root cause of the issue. She has a passion for PCOS and thyroid health, and truly believes that treating hormone-related conditions is not a one size fits all form of medicine. As someone with PCOS, she has seen firsthand the success of treating PCOS with functional medicine.

Dr. Lyndsey recently got married on John’s Island in December, and lives with her husband and sweet puppy dog, Mabel. You can find her out in the boat on the weekends exploring the creeks of the Stono River, or cooking up a delicious meal in her kitchen for her family and friends. As a Charleston native, she is so excited to be able to give back to the city she loves so much.
